The LHIP's CHWs help individuals to apply for medical and dental insurance coverage. We provide assistance to apply for State subsidized insurance plans, such as Mass health, Connector Care, etc. We also provide support for individuals to enroll on unsubsidized plans (private plans) when they are not eligible for other programs in small businesses. Here at LHIP we offer evidence based education programs, such as the Chronic Disease Self- Management Program. The Spanish version is called Tomando Control de su Salud. Participants learn important skills, such as nutrition, how to manage their chronic diseases, and how to communicate with their doctors. All of the participants who have attended the Mi Vida, Mi Salud workshops reported they were “very satisfied” with the program, and many highlighted their better understanding of nutrition labels and their weight management and control.

As part of Latino health Insurance program (LHIP) efforts to address the social determinants of health and the health care needs of Hispanic and Latino patients with diabetes, LHIP created a Diabetes Prevention Clinic. Dr. Milagros’ approach was to start by looking at how people receive health care information, how they are engaged in their communities and what works in terms of preventing or managing their chronic conditions. The clinic provides care management and care coordination to individuals with diabetes or prediabetes by connecting them to resources available within LHIP or offered by community partners in order to reduce complications from their diseases and the incidence of unnecessary emergency room visits and hospital admissions.
We provide Covid-19 and Flu vaccinations and clinical follow up and treatment. LHIP's patient navigators have been valuable to this work, conducting outreach, providing resources, and assisting patients to register for vaccinations and/or for Medicare, MassHealth, and SNAP. The patient navigators are multilingual and are trained to communicate important information and build trust. Type 2 diabetes is common, is serious, and with lifestyle changes can be prevented or delayed. If you have prediabetes, the National DPP lifestyle change program is one of the most effective ways to prevent type 2 diabetes. In the program, you'll learn skills to help you lose weight, become more active, and improve your overall health. See the articles page for more information. The Latino Health Insurance Program is educating men about the risk of prostate cancer. If you are a man forty years or older and have not had a screening test, please reach out to us. We will provide a screening and discuss your risk to help learn about and prevent prostate cancer.
